How to remove/cancel a phone from my account

My mother passed away. I would like to remove her phone from my account. How do I proceed?

I am not trying to close my entire account. I have internet billed seperately still for some reason and 2 Phones for 35 dollars a month each. I am just trying to stop service for the phone that I bought for my Mom and put her on my bill. This phone is not being used as she is deceased. I just want to stop service on her phone and keep my plan intact, minus one phone.
