Charged for data when cellular data is disabled

We have an iphone for an elderly family member who doesn't use cellular data.
So cellular data is disabled on the phone. There are no app exceptions set yet our bill is still showing data usage for that device amounting to an extra 50 dollars on the bill.
It looks like the app store updates used cellular data. Does the iphone ignore cellular data being off to download app updates when you're away from home?

As a matter of privacy no representative is going to be able to see what is using the data on a phone; only the amount used, a rough time stamp, and any associated charges. It is however true that iOS in particular can supersede Cellular Data being disabled for certain system level functions.
Apple has a useful guide on their Support website where they go over in detail how to View or change cellular data settings on iPhone, and how to set your preferences for each app specifically.
Otherwise, you can also check out this Apple support page on How to view how much data you're usingto narrow down what may or may not be using the data in question.
